Admitting a Patient (No Equipment)

How to Admit a Participant (No Equipment):

  1. Care Coordinators will login to the TouchPoint Care portal with the Account name, personal Username, and Password.

  1. In the Navigation bar, on the left side of the page, select “Participants”, then “All Participants”.

 

  1. If the patient has already been loaded into the portal, locate the patient within the participant list. Choose the appropriate patient by clicking on the patient’s name within the list. This will bring you to the “Participant Information” page.
  2. If the patient has not been added to the portal yet, you will need to click on the orange “Add Participant” box in the upper righthand side of the page. This will bring you to the “New Participant” page.

 

  1. On the “New Participant” page, complete the process of filling in the patient’s demographic “Details” and input all the pertinent data. Select who the alerts should be sent to by selecting on the up/down arrows on the "Send Email Alerts to" field. This will generally be the participant's case manager. 

 

  1. If the patient has a smartphone or a cell phone with texting capabilities, be sure to input the cell number in the “Primary Phone” box.

 

  1. Click the plus sign (+) to open each heading and continue to input the patient’s information within the “Contact Information”, “Emergency & Medical Contacts”, and “Personal Information” sections. Once completed, select “Save & Assign” (step a.). 

 

  1. Once saved, you will be taken to the “Assign Groups” page. To follow those steps and learn more about that, click here.

 

 

NOTE:

  • Patients must be added into TouchPoint Care by the Care Coordinator or they can be uploaded via the EXCEL spreadsheet provided to your organization.
  • If you are utilizing risk scoring, you will go through a series of questions that will be utilized to calculate the participant’s initial risk score.

 

 

Adding Participants from an Interface or Data file: 

  • TouchPoint Care has developed an interface to electronically load participant data into the system from an XML file and “pushed” to the touchpointcare.net application via Secure FTP.  
  • TouchPoint Care will poll the SFTP server periodically and when an XML file is detected, it is processed. 
  • The Participant Interface can automate adding Participants to a group.  
  • The Interface also allows you to Inactivate and Activate participants are required.
