Located under the "Help" section, you will locate a "Contact TeleMed" button. This will direct you to a form to fill out to receive assistance from TeleMed.
Always select "Customer Service" as who the message needs to be sent to. In your message, please be as detailed as possible regarding what you may need.
IMPORTANT: Ensure to always list your email address in the "From" field. If you do not include an email address here, TeleMed will be unable to reply to your inquiry.
