TriagePro and Risk Scoring Configuration

To access TouchPoint Care, go to https://www.touchpointcare.net/

Care Coordinators will login to the TouchPoint Care portal with the Account name, personal Username, and Password.

Once logged in, you will be on the landing page and can choose from the different actions by hovering over the menu on the left side bar.

The Concept:

TriagePro provides automated scoring and sorting to prioritize the most critical patients at the top of your Dashboard. TriagePro calculations are customizable to your organization based on your program and patient population.

 

TriagePro Feature Activation:

To turn on TriagePro:

1. Navigate to "Company" and select "List Providers" from the menu on the left. 


2. Select the provider for which you will be configuring the TriagePro.

 

3. The Provider's Information will come up. On the "Details" tab, open the "Settings" section by clicking on it. 

 

4. Set "TriagePro" to "Basic". If you would like the Dashboard to default sorting to the highest TriagePro score, check “Sort dashboard participants by TriagePro score”.   

    • When TriagePro is set to Basic and utilizing automated calculations, the user will not be able to manually update the TriagePro score.  

 

To show the TriagePro score on the Dashboard:

1. Navigate to "Company", then select "List Providers".

 

2. Select the Provider.

 

3. Go to the "Dashboard" tab and check "Display TriagePro Score", then click on the orange "Save" button on the top right. 

 

Deactivation:

If you would like to disable TriagePro you can change the TriagePro setting in the Details section of Provider Information to "Not Used". If this change is made, all existing TriagePro scores will be removed so that clinicians are not making triage decisions based on stale scores. If you would like to reactive TriagePro, simply change the TriagePro setting back to "Basic". If TriagePro is reactivated, all scores will be recalculated based on the most recent data and displayed to clinicians. 

 

Scoring:

TriagePro scores are calculated based on points set in IVR Question Settings.

To access IVR Question Settings:

1. Navigate to "Questions" and select "IVR Question Settings".

 

2. Select the Series.

 

3. Select the Question you want to update.

4. Go to the "TriagePro Analytics" tab.

    • TriagePro scoring is currently supported for numeric, 10 point scale, and yes/no question types. 

 

In TriagePro Analytics you can assign a point value to different responses. All questions that have TriagePro Analytics available in the IVR Question Settings page are eligible to be included in the score. The TriagePro score assigned to a participant is a percentage of the total possible points across all questions or vitals that the participant answered and that had points assigned.    

We recommend adding scores on a 10-point scale to standardize points entry and scales as TriagePro scores are calculated based on a percentage of total available points.

 

Scoring Algorithm:

TriagePro scores are based on a participant’s total points triggered vs. total points available for the readings where they have triggered points.

Examples below:

1. One reading triggered TriagePro scoring

a. Highest points that could be assigned for a temperature reading is 10.
b. A patient’s reading is 102, which assigns 5 points.
c. f no other readings or question responses have triggered TriagePro points, the patient’s score would be 50. This is calculated from the patient’s points accounting for 50% of the total available points for triggered questions.

2. Another reading for the same patient triggers additional TriagePro scoring

a. Highest points that could be assigned for an oxygen saturation reading is 10.
b. A patient’s reading is 92, which assigns 7 points.
c. The patient’s score would be 60. This is calculated from the patient’s points, 12 total out of 20 points, accounting for 60% of the total available points for triggered questions.
d. The patient takes another reading for the same vital recorded previously.
e. The patient takes another oxygen saturation reading of 95, which changes their 7 points assigned from the previous reading to 5.
f. The patients TriagePro point total is now 10 out of a possible 20 points. The patient’s score would be 50, reflecting 50% of the total available points for triggered questions.

 

Patient Discharge and Reactivation:

When a patient is discharged, their TriagePro score will remain on the patient record. If that patient is then readmitted, the score will be cleared out and recalculated based on the readings and answers recorded only for that episode of care.
